Documents required to support a claim for:
Unauthorised Transaction from TNG Digital Wallet
- Screenshots of your mobile setting;
- Bank statement of unauthorised transaction; and
- Highlight unauthorised transaction based on transaction history etc.
Accidental Death (death due to accident, accidental death due to road traffic accident or in public conveyance)
- Police report;
- Hospital certificate;
- Post-mortem report;
- Death certificate;
- Copy of the Identity Card;
- Grant of Probate (GP) / Letter of Administration (LA); and
- Other additional documents (to prove that the accident is arising from a road traffic or in public conveyance).
Loss of Personal Belongings due to Theft
- Police report(s); and
- Proof of value and ownership of lost item.
Unauthorised Debit / Credit Card Spending due to Theft
- Police report(s); and
- Copy of statement(s) and investigation report(s) issued by the debit/credit card issuing bank showing the details of unauthorised use or spending of debit/credit card that belongs to the Insured as the principal cardholder, and the total loss amount to be borne by the Insured where such claim is being rejected by the said issuing bank.
Note: In the event of the person making the claim (claimant) is not the Policyholder or Insured, proof of the claimant’s identity and proof of relationship between the Insured and the claimant must also be submitted.
