- Critical Illness Attending Physician’s Statement
- A copy of Insured Person’s Identity Document (NRIC or Passport)
- 5 copies of the Consent Form and Patient’s Appointment Card
- All medical test results including MRI/CT scan
- A copy of Claimant’s Identity Document (NRIC or Passport) (if claimant is other than Insured Person)
- Original itemised bills (Invoice/Statement) and original official receipts including deposit receipts

- Blood Test Result
- Cardiac Enzymes Test Results (CKMB, Troponin)
- Electrocardiogram (ECG)
- Angiogram Report
- Angioplasty (PTCA) Report
- Coronary Artery Bypass Graft (CABG) Report
Serious Coronary Artery Disease:
- Blood Test Result
- Cardiac Enzymes Test Results (CKMB, Troponin)
- Electrocardiogram (ECG)

Cancer:
- Blood Test Result
- Histopathology / Biopsy Results
- Radiology Report e.g. MRI, CT Scan, PET Scan, Ultrasound, X-Ray

Stroke:

Kidney Failure:
- Dialysis Card / Treatment Receipts
Lifestyle Assistance Allowance Benefit:
- Receipts or other form of proof of engagement to home care, hospice care, palliative care and/or home cleaning services. These documents are to be provided each month.
