How do I make a claim for Critical Illness plan?

1. You need to notify us by submitting your claim within 60 days from the date of diagnosis.
2. Obtain the claim submission webpage link either in the email you received your e-certificate or Touch ‘n Go eWallet > Insurance > ‘My Policies’.
3. Fill in the Attending Physician’s Statement (APS) form of respective critical illness, where applicable.
4. Complete the claim submission webpage and provides the APS and supporting documents required.
5. Submit your claim to AIA via the claim submission webpage.

Payments for benefits will be made by AIA directly. Payment for all benefits will be paid to the Insured Person.

• You may obtain the Attending Physician’s Statement (APS) for Heart Attack or Serious Coronary Artery Disease, Cancer, Stroke and Kidney Failure as provided in these links.
